us-passport4A passport application form is one of the most important pieces of paper you will need if you are interested in getting a U.S. Passport. This form tells passport officials everything that is needed in order to qualify you for a passport. Take the time to understand this important document which is the first step towards receiving your passport.

The United States Department of State requires that all first time passport applicants complete form . The form can be picked up at any of the 8,000 passport processing centers throughout the U.S. If you live in an urban location, you're likely to have a wide selection of stations in your city, and these not only carry the applications but are also an authorized processing center.

The form itself is pretty straight forward. The sections are broken up into proof of United States citizenship, proof that you are who you say you are, the need for color passport photographs and what to expect to pay in processing fees. Some interesting and relatively new information is in the section titled "Electronic Passport Statement." This section discusses a new passport that's being issued which has an electronic chip within the passport. This new type of passport adds additional protection for security issues as well as faster transfer through the port-of-entry locations which are set up to scan the electronic passport.

When you pick up or print your , you'll notice that it is pretty self explanatory regarding the information to be entered. Be sure to use black ink and print very carefully, as any illegible or smudged items can easily cause your application to be rejected. Pay attention, also, to the format for dates which is required to be in the form "mm/dd/yyyy" which is a 2 digit month, 2 digit day and 4 digit year.

The 2x2 square box on the left middle area of the form is for your passport photo. These photos must be taken by an authorized passport photography location. Most passport processing centers also take passport photos so check with the office you've selected to see if they provide this service.

You'll notice that additional documentation is required to be submitted with your passport application form. Since your passport is a verification of your identity, items such a driver's license or military ID are required. Additionally, you will be required to submit proof that you are a U.S. citizen such as an original certified Birth Certificate, Naturalization Certificate or Certificate of Citizenship. These will be returned when your passport is mailed to you, which is typically 2 to 3 weeks.
